Online Access through the Patient Portal is HIPPA compliant and secure.
If you are an existing patient, we have a free service available called the "Patient Portal." This link allows you to obtain your lab results, immunization and medication lists.
To use the Patient Portal, you utilize a username and password. You must have a permanent email address that you check consistently.
NOTE:
- WHEN SETTING UP YOUR PORTAL BE SURE TO CHOOSE DR. DAVOODI AS YOUR PROVIDER.
- If you prefer an app is available for Ipad/IPhone use.
- When you add an item to your FollowMyHealth® Universal Health Record, it is not being added to your provider's electronic medical record. To make permanent changes to your legal health record please contact our office.
